- Fees to Department of State: check or money order payable to US Department of State (amount varies depending on item requested)
- Fees to Union County Clerk’s office, $35: payable by cash, check, money order or credit/debit. Credit/debit is subject to a 3.5% processing fee
- For children 15 & under only: both parents must be present with photo identification, or one parent present with the Department of State’s consent form completed and notarized by the other parent.
