Passports

The Passports division receives passport applications, reviews, certifies new passport applications, and sends them to the Department of State for issue. Passport applications are executed at Union County Courthouse and forward to the appropriate facility for processing. Photos may be obtained from CVS or Walgreens in Lake Butler.

*For passport renewals only: If certain Department of State criteria are met, individuals may mail a renewal application (Form DS-82) directly to the Department of State and will not incur the $25.00 fee to the Clerk of Court. To determine eligibility for mail-in renewal, visit the Department of State’s passport renewal page by clicking here. (Note: The Clerk of Court is not permitted toexecute Form DS-82 mail-in renewal forms).

All forms must be filled in using black ink only.
