Passports

The Passports division receives passport applications, reviews, certifies new passport applications, and sends them to the Department of State for issuance. Passport applications are executed at the Union County Courthouse and forwarded to the appropriate facility for processing. Photos may be obtained from CVS or Walgreens in Lake Butler.

Passport Checklist

  • Birth Certificate. It must be an official copy with a raised seal.

  • Application. It can be unsigned, completed with black ink, or completed online and printed.

  • Photo Identification

  • 2 x 2 Passport Photo

  • Fees to the Department of State. Checks or money orders should be made payable to the US Department of State. The amount varies depending on item requested.

  • $35 fee to Union County Clerk’s office, payable by cash, check, money order, or credit/debit. Credit/debit is subject to a 3.5% processing fee.

Children Ages 15 & Under

Both parents must be present with photo identification, or one parent present with the Department of State’s consent form completed and notarized by the other parent.

Important Information

Passport Renewals

If certain Department of State criteria are met, individuals may mail a renewal application (Form DS-82) directly to the Department of State and will not incur the $25.00 fee to the Clerk of Court. To determine eligibility for mail-in renewal, visit the Department of State’s passport renewal page by clicking here.

Note: The Clerk of Court is not permitted to execute Form DS-82 mail-in renewal forms.

All forms are available at our office location. You may also download forms from the U.S. Department of State website. All forms must be filled in using black ink only.
